Altriva was founded in Richmond, Virginia by a PM&R physician who spent years working in facility-based settings across Virginia — IRF, SNF, and acute care hospitals — and lived every documentation and billing pain point these products solve. We exist to give physicians their time back, and to build tools that are actually designed by people who use them.
Built Because the Tools That Existed Weren't Built by People Who Actually Use Them
Dr. Randolph Jenkins is a PM&R physician with a facility-based practice spanning inpatient rehabilitation facilities, acute care hospitals, and skilled nursing facilities across Virginia. He was managing complex multi-diagnosis patients, fighting commercial and Medicare Advantage IRF denials, and spending hours every evening completing documentation that generic tools couldn't handle correctly.
The ambient scribes on the market were built for primary care. They transcribed words but couldn't surface comorbidities, couldn't generate IRF medical necessity language, and couldn't produce the structured, defensible documentation that facility-based PM&R requires. More cleanup than they saved.
The charge capture tools were built by engineers and billing companies — not by physicians who use them. Every tool required excessive clicks, manual re-entry, and a billing manager to manage what should be a physician-controlled workflow.
