Altriva was founded in Richmond, Virginia by a PM&R physician who spent years working in facility-based settings across Virginia — IRF, SNF, and acute care hospitals — and lived every documentation and billing pain point these products solve. We exist to give physicians their time back, and to build tools that are actually designed by people who use them.
Dr. Randolph Jenkins is a PM&R physician with a facility-based practice spanning inpatient rehabilitation facilities, acute care hospitals, and skilled nursing facilities across Virginia. He was managing complex multi-diagnosis patients, fighting commercial and Medicare Advantage IRF denials, and spending hours every evening completing documentation that generic tools couldn't handle correctly.
The ambient scribes on the market were built for primary care. They transcribed words but couldn't surface comorbidities, couldn't generate IRF medical necessity language, and couldn't produce the structured, defensible documentation that facility-based PM&R requires. More cleanup than they saved.
The charge capture tools were built by engineers and billing companies — not by physicians who use them. Every tool required excessive clicks, manual re-entry, and a billing manager to manage what should be a physician-controlled workflow.
So Dr. Jenkins built the tools he wished existed. MacroMate Pro for documentation completeness, clinical decision support, and payer defensibility. Codana for charge capture and billing submission that a physician can manage themselves — without a billing manager.
"I built MacroMate Pro because the tools that existed weren't built by people who actually use them. Every feature exists because I personally experienced the problem it solves."
To eliminate the administrative burden that drives physician burnout — starting with the two most consequential pain points in facility-based PM&R practice: documentation completeness and billing control. Built by a physician who lived both problems.
Every product decision is made by asking: does this make a clinician's life better? We build for the physician at the bedside, not the administrator at the spreadsheet.
Generic AI tools fail in specialized medicine. MacroMate Pro is built for the specific complexity of facility-based PM&R — not adapted from a primary care tool.
We believe independent physicians are the backbone of American healthcare. Our pricing, support, and product philosophy are designed to empower solo and small group practices.
AI in healthcare must be transparent, auditable, and controllable. Altriva's AI surfaces recommendations — physicians make decisions. Always.

Board-certified PM&R physician with a facility-based practice spanning inpatient rehabilitation facilities, acute care hospitals, and skilled nursing facilities across Virginia.
Dr. Jenkins built MacroMate Pro and Codana because the tools that existed weren't built by people who actually use them. Every feature in both products traces directly to a problem he experienced at the bedside — managing complex multi-diagnosis patients, defending IRF admissions against commercial denials, and spending hours on documentation that generic tools couldn't handle correctly.
His clinical practice remains active. He uses MacroMate Pro and Codana daily — which means every update, every new feature, and every design decision is tested in the real world before it reaches you.

Brings extensive leadership experience spanning both the automotive industry and healthcare — two sectors that share more operational DNA than most realize. Jake bridges those worlds creatively, applying proven frameworks from automotive operations to healthcare delivery: driving tech adoption, streamlining workflows, and building the operational infrastructure that allows Altriva to deliver at scale.
